Team Leader – Creative Support with Creative Support in Leeds, West Yorkshire is a people-focused role involving work to join us as a Team Leader at Extra Care service in Leeds.

Responsibilities: The advert points to lead the coordination and management of our person-centred support for older adults, develop positive professional relationships with individuals, and offer support to people both in their own flats and in the well-resourced communal facilities, lead the staff team to provide the highest standards of customer care and person-centred service delivery to promote independence, choice and wellbeing, and undertake an assessment of a prospective tenants’ care and support needs and preferences To maintain excellent communication with family members and significant others and to promote family involvement in the ongoing delivery of Personal Support Plans and activities To support tenants to maintain personal and life skills and to work with them to establish a positive lifestyle and routine which includes social, leisure and recreational activities To provide staff with personal support, coaching, informal/formal supervision and to undertake regular appraisals, with emphasis on safe, respectful support.

Requirements: Suitable applicants may bring a minimum of 3 years’ experience and relevant qualifications are essential and a passionate, inclusive, and anti-racist organization – Stonewall Diversity Champion, Disability Confident Employer who have recently received Investors in People Gold awarded.
