Responsibilities: The advert points to include leading a team of staff, providing supervision, coaching, performance management, and support to the staff team, maintain their independence, enhance their wellbeing, and enjoy opportunities for personal development, referrals, undertake initial assessments of need, and ensure all service users have co-produced support plans and designated Key Workers, and Working closely with local multi-disciplinary teams and community-based agencies, the post holder will provide holistic, wrap-around support, with emphasis on safe, respectful support.
