Responsibilities: The advert points to help facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need sort term support due to a change of circumstances The Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care co-ordinators, reablement staff, discharge facilitators, AHP staff as well as representatives from different services, work with a range of professionals and people to support the principles of Home First, work as part of a team in a pro-active manner identifying opportunities for timely discharge whilst ensuring the patient has the appropriate level of support, which may incorporate the use of technology upon discharge from hospital, and have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments, with emphasis on safe, respectful support.

Requirements: Suitable applicants may bring the worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for timely discharge whilst ensuring the patient has the appropriate level of support, which may incorporate the use of technology upon discharge from hospital, The worker will be required to undertake essential training including in adult support and protection training, The worker must have the ability to travel throughout West Lothian, and The skills and knowledge candidate must have: - SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential.
