Job Description Link Worker (13/874M) (Fixed Term) Kirkconnel £25,613.33 - £27,197.33 The Learning Disability Service provided by Dumfries and Galloway’s Health & Social Care Partnership supports young people and adults who have a range of needs including learning/ physical disabilities and autism spectrum conditions. As a Link Worker within Day Services, the post holder will be part of a team committed to delivering high quality person-centered services based on the principles of self-directed support, ‘The Keys to Life’ and Health & Social Care Standards. The post holder will support people to access community-based opportunities to develop skills, promote independence, improve health and wellbeing and participate in adult learning/work-related activities.

Further context from the advert indicates that reporting to a Support Coordinator, the post holder will provide direct care and support and contribute towards personal planning and reviews, team meetings and recording of electronic information. In this post the post holder will be required to work 32 hours per week 52.14 weeks per year. Some evening and weekend work may be required.
